U.S. reports a significant increase in measles cases, reaching levels not seen since the disease was alleged to be eradicated.

In 2025, the United States is grappling with a significant increase in measles cases, with over 1,277 confirmed cases surpassing the record from 2019[1]. This surge, particularly in states like Texas, has been attributed to several factors, including lower vaccination rates, the highly contagious nature of measles, and the global rise in measles activity.

One of the primary reasons for the decline in vaccination rates is a drop in immunization coverage among kindergarteners. The percentage of kindergarteners vaccinated against measles dropped from 95.2% during the 2019-2020 school year to 92.7% in the 2023-2024 school year[1]. This decrease leaves communities more susceptible to outbreaks, particularly in areas where populations are unvaccinated or under-vaccinated.

The Texas outbreak, for instance, primarily affects unvaccinated children and teenagers[1][4]. Tragically, there have been measles-related deaths among this demographic. Less than 1% of confirmed cases are actively infectious, indicating efforts to contain the outbreak[4]. However, the need for higher vaccination rates remains a critical public health priority.

Measles, one of the most contagious human diseases known, can easily spread in communities where people are unvaccinated or under-vaccinated[2]. Additionally, travel-related cases play a role in the ongoing outbreaks. Although measles was declared eliminated in the U.S. in 2000, ongoing cases are partly due to unvaccinated international travelers bringing the virus back into the country and spreading it to unvaccinated communities[2].

Following a decrease during the COVID-19 pandemic, measles activity is rising globally, which increases the risk of introduced cases in the U.S.[2]. To combat this, the CDC has advised that anyone traveling internationally should be vaccinated with two doses of the MMR vaccine[5].

In response to the outbreak, many affected counties have introduced expanded vaccination guidance, allowing infants to get their first shot as early as 6 months old instead of waiting until 1 year[6]. Special vaccination clinics have led to additional coverage for thousands of people in response to the outbreak.

In Texas, nearly 1 in 4 kindergartners did not have their required MMR vaccine in the 2024-25 school year, one of the worst rates in the state[7]. However, early vaccination rates among infants in Texas have significantly increased, with MMR vaccination rates among 6-month-olds more than eight times higher than in 2019[8].

The majority of cases in the US this year have been in people who are unvaccinated[9]. Three people have died from measles in 2025, all of whom were unvaccinated[10]. Mexico has issued a warning for people traveling to the US and Canada due to high measles case rates[11].

In conclusion, the combination of lower vaccination rates, the highly contagious nature of measles, and the global rise in measles cases contributes to the increase in U.S. measles cases in 2025. Public health initiatives and increased vaccination rates are crucial to controlling and eventually eradicating this preventable disease.
